xF2 Unmaintained ACP - top logout button, with user name 1.0.0

Nothing's changed, I've finally managed to upgrade my forum to the latest 2.2.8 Patch 1 release. This is a very simple addon and still works fine so I thought I'd get rid of the test status and dive in with the 1.0.0 version for completeness.
