[TH] Self Delete - Allow users to delete their own account

Description

This add-on adds an option for users to request to delete their own account.

Features
  • Users can choose to delete their own account
  • ACP page where admins can see all reasons for leaving that users have entered
  • Multiple layers of protection from accidental account deletion
    • Confirmation overlay
    • Requirement for a reason to be entered
    • Check-box which must be ticked
    • Link in a confirmation email
    • 24-hour window during which the user can change their mind
